What could it be if my elbow was bruised for over a year?

My elbow was bruised for almost 2 years. You can not see However, the bruise. It hurts if you push on it, and if I put it on a hard surface like a desk, it's so bad! I want to scream! I no idea what caused this. It hurts really bad and I want to know what it is if I can handle it. Thank you!
